One of the many commendations you can complete during The Lair of LeChuck Tall Tale is the “Second Biggest Monkey Head” commendation. You can start working on the commendation as soon as you arrive at Monkey Island during this Tall Tale.

If you are just looking to complete “Second Biggest Monkey Head” and use the Cotton Swab Key, your first step is to make your way to Monkey Island!
Second Biggest Monkey Head Commendation
To complete the “Second Biggest Monkey Head” commendation and use the Cotton Swab Key, you’ll need to insert the Cotton Swab Key in the left ear of the Stone Monkey Head. However, before you can do that, you’ll need to get the Cotton Swab Key by placing all three Monkey Idols on the Monkey Hand. Once you put all three Monkey Idols on the Monkey Hand, it will raise and reveal the Cotton Swab Key.
Three Monkey Idols
The three Monkey Idols are the Buried Monkey Idol, the Caged Monkey Idol, and the Totem Monkey Idol. You can find the Buried Monkey Idol’s location in the Buried Idol Map, held by the hanging skeleton. The Caged Monkey Idol is hanging from the mast to the West. And the Totem Monkey Idol is on top of the tall Monkey Totem in the jungle.
To find the Buried Monkey Idol, you must solve several puzzles involving Herman and his rowboat. You can reach the Buried Idol Map from the hanging skeleton with Herman’s rowboat.
To free the Caged Monkey Idol, you must solve several puzzles involving Hermans’ Stockade and the sun’s reflection. You can ignite a Gunpowder Gummy with the sun’s reflection and blow open the cage.
To reach the Totem Monkey Idol, you must solve several puzzles involving Herman’s Primitive Art. With the Primitive Art, you can knock down the totem.
